Lutheran Services Carolinas is a faith-based health and human services organization serving children, families, and seniors in the Carolinas.

Who we are
Lutheran Services Carolinas assists and advocates for those we serve, both young and old, who need obstacles removed and pathways cleared on the way to a more fulfilling life.
LSC is the result of a 2012 partnership between two social ministry organizations, Lutheran Services for the Aging and Lutheran Family Services of the Carolinas. Before these two organizations joined forces, LSA offered programs for seniors, while LFS provided family services, including foster care and adoption.
Senior services
LSC offers a variety of living options and healthcare services for North Carolina seniors. Regardless of need, we meet the seniors in our community wherever they are and provide them with a life of fulfillment, fellowship, and professional care.

Rental apartments, retirement communities and continuing care retirement communities, including cottages and apartments on campuses designed to carry you forward if your needs change.

We know that the people who work directly with our senior residents every day are the ones who need to serve them best. The intensive New Pathways training program creates more capable LSC staff members.

Heartful Living is our innovative approach to dementia care designed to meet residents where they are, affirm their experience, and support meaningful engagement throughout daily life with roots in dignity, purpose, and compassion.

Twenty-four-hour skilled nursing care and state-of-the-art rehabilitation services to short-stay guests, while also offering long-term residents quality care and quality of life.

Adult day service programs, including LSC's Trinity Living Center in Salisbury, help end the social isolation that adults can experience due to changes in health, resources, or mobility.

Trinity at Home gives seniors the freedom to remain comfortably in their own homes. PACE is an innovative Medicare program and Medicaid option for adults 55 and over who need skilled nursing care but wish to remain at home.

Life at an LSC community
LSC's newest senior living facilities are purposefully designed to provide a more contemporary environment for residents. Features include small neighborhoods, private rooms, and community features like Internet cafes, buffet-style dining, and playgrounds for visiting children.

We invite you to learn more about the specific services and benefits of each LSC community. Four offer independent living alongside health care options. Seven focus on health care, from skilled nursing and rehabilitation to assisted living and specialized memory care.
Trinity Oaks, Trinity Elms and Trinity Landing are licensed by the North Carolina Department of Insurance as continuing care retirement communities.

LSC follows a person-centered model of healthcare called New Pathways. New Pathways encourages the development of resident-centered practices and a culture that emphasizes an individual's independence and overall quality of life. In essence, daily decisions, both large and small, are left up to the residents of our communities as much as possible.
The intensive New Pathways training program creates more capable LSC staff members, giving them the competencies and compassion to make the best decisions for the residents in their charge.
At Lutheran Services Carolinas, we understand that dementia influences how individuals experience the world each day. Heartful Living is our innovative approach to dementia care designed to meet residents where they are, affirm their experience, and support meaningful engagement throughout daily life with roots in dignity, purpose, and compassion.
Developed in partnership with Dementia By Day, the program specializes in customized, informed practices for long-term care. Central to the Heartful Living approach is the philosophy of affirming whatever is real for the resident in the moment. Caregivers learn to respect and validate each individual's experience rather than correction or reorientation, a strategy that can reduce stress and foster connection.
Three of our communities are licensed by the North Carolina Department of Insurance as continuing care retirement communities: Trinity Oaks in Salisbury, Trinity Elms in Clemmons, and Trinity Landing in Wilmington. Each appears on the Department's register of licensed CCRCs under Rowan, Forsyth and New Hanover counties respectively.
A continuing care retirement community means you can move in while fully independent and remain on the same campus if your care needs change. Our other communities offer independent living, assisted living, memory care, rehabilitation or skilled nursing without that continuing care contract.
PACE stands for Program of All-Inclusive Care For The Elderly. It is an innovative Medicare program and Medicaid option for adults 55 and over who need skilled nursing care but wish to remain at home. The PACE focus is on prevention, early intervention, and wellness, resulting in overall health improvements and cost savings.

Adult day service programs, including LSC's Trinity Living Center in Salisbury, help end the social isolation that adults can experience due to changes in health, resources, or mobility. Participants enjoy activities and establish friendships while receiving medical care, nutritious meals, and assistance with personal care if and when it is needed.
Caregiver support and respite are another fundamental part of adult day services. Adult day services provide an alternative to assisted living or a live-in senior community. Families know their loved ones are safe and secure while they are able to work, rest, or run errands.

Alongside senior services we offer foster care, adoption and transitional living for youth and young adults, adult programs including supported living, host homes, mental health services and traumatic brain injury services, the New Americans Program, and disaster response across the Carolinas.
LSC is an open book. Our website shares detailed information about all our services, along with our audits and IRS filings, which are publicly accessible. We publish our 990s, a community stewardship report, a corporate compliance policy and a cultural competence plan, and LSC carries a transparency seal from Candid.
